The Care@Home approach to patient care

Learn about the Care@home pharmacy model in Tasmania and the virtual services supporting both inpatient and outpatient care. This session covers the Virtual Care Pharmacy Manager role, which provides consistent support across Tasmania's three regions through virtual pharmacy hubs. You'll explore how this approach supports the new Hospital@home framework, the next phase in evolving the traditional hospital in the model. It integrates rapid response outpatient care and HitH inpatient services within a single framework. Medication management and support across this service are essential, particularly in terms of medication governance, access, and logistics.
Complementing this is the Care@home model, which supports acute medical care, chronic disease coaching, and after-hours palliative care. It offers patients a pathway to access the most appropriate care option, whether through a Care@home outpatient/community model or another service within the broader health system.

This presentation will address the following learning objectives:

  • Describe the role of pharmacy in supporting virtual care models that span inpatient and outpatient services
  • Explain how integrated care frameworks can enhance continuity, responsiveness, and patient-centred outcomes in home-based healthcare
  • Identify key considerations in medication governance, access, and logistics within decentralised and digitally enabled care environments
  • Evaluate pharmacy’s contribution to acute care, chronic disease management, and palliative care delivered outside traditional hospital settings
  • Apply principles of virtual service design to improve medication management and support across diverse care pathways
